As a condition of being allowed to participate in activities organized by Wildlife Jewels® ), I, the Participant, agree to the following: 1. THE PARTICIPANT: I am volunteering my services for Wildlife Jewels®. I will perform assigned tasks that are within my physical capability and will not undertake tasks beyond my ability. I will perform only those tasks assigned, observe all safety rules, and use care in the performance of my assignments. 2. ASSUMPTION OF RISKS: The Wildlife Jewels® Program activities consists of field trips, beach cleanups, wildlife watch and monitoring which might bring the Participant into contact with wildlife and potential hazardous environment. I acknowledge that participating in beach or inland water cleanups, hiking, wildlife rescue and monitoring carries inherent risks, including but not limited to the potential for injury, infection, or physical harm from hazardous materials, wildlife encounters, poisonous plants, and environmental conditions such as flooding, rockslides, sun exposure, or rugged terrain. I hereby voluntarily assume all risks and responsibility for injury, death, or property damage foreseeably arising from Program activities even if not expressly stated above. 3. PHOTOGRAPHS, VIDEOS, AND/OR AUDIO RECORDINGS CONSENT RELEASE: I, as Participant, consent that my photographs/video/audio may be taken, or my image digitally recorded while engaged in volunteer activities for Wildlife Jewels® and consent to the use of my image in published materials and websites that promote the programs and activities of Wildlife Jewels®. 4. WAIVER, INDEMNITY AND RELEASE: I, as Participant, waive, release, discharge, and hold harmless Wildlife Jewels® from any and all claims, damages, losses, death, and any actual or alleged personal injury caused in whole or in part by any act or omission arising from my participation in the Program, including both claims arising during the activity and after I complete the activity. I accept sole responsibility for my own actions in the event of injury, damage, or loss. 5. PARTICIPANT ACKNOWLEDGEMENT: I, the Participant, have received this RELEASE OF LIABILITY AND ASSUMPTION OF RISK CONSENT in advance of the Program, have read it before signing below, and fully understand its contents, meaning and impact and that it is binding on me, my heirs, representatives, administrators and assigns. I, the Participant, realize I cannot withdraw my consent after I sign this RELEASE OF LIABILITY AND ASSUMPTION OF RISK CONSENT. If the Participant is UNDER 18 years old: I am the parent or legal guardian of the Participant. I have the authority to allow the Participant to participate in the Program, and I sign this RELEASE OF LIABILITY AND ASSUMPTION OF RISK AND PHOTOGRAPHS/ VIDEOS/AUDIO RECORDINGS on behalf of the Participant. By providing my email address below, I agree to receive future communications from Wildlife Jewels® regarding Wildlife Jewels® events and other Public Education programs. I have fully informed myself of the contents of this liability release and assumption of risk.
